I, hereby agree to indemnify, defend and hold harmless the Ladies Professional Golf Association, and all sponsors, volunteers, agents, employees, representatives, volunteers and assigns from any and all claims that I (or my representatives, assignees, successors or heirs) may have now or at any time in the future, as a result of any injury arising out of or any third party claims resulting from my participation, including any injuries or claims arising from COVID-19, in the LPGA Lesson Zone

I acknowledge the contagious and harmful nature of COVID-19 and assume all risk of exposure or infection by participating. I acknowledge and agree that I will be subject to temperature checks, questionnaires and must practice required protective measures by the Tournament including social distancing and masking if and when mandated. I confirm that I am not now infected with COVID-19 or experiencing symptoms of the same.
